These Terms of Service ("Terms") govern your access to and use of ClearSignal Health, a wellness information service operated by Finnan Wellness LLC, doing business as ClearSignal Health ("ClearSignal," "we," "us," or "our"). By purchasing or using our services, you agree to these Terms in full. Please read them carefully.

1. Nature of Service

ClearSignal Health provides an informational wellness service that includes facilitated lab testing and a written specialist review prepared by Erica Finnan, MSN, APRN-C. Our service is designed to help you better understand your lab results in the context of functional and optimal health ranges.

ClearSignal Health is not a medical practice. Our services are educational and informational in nature only. Specifically:

• ClearSignal does not provide medical advice, diagnosis, or treatment.
• Use of ClearSignal does not create a patient-provider relationship between you and Erica Finnan, Finnan Wellness LLC, or any affiliated clinician.
• The written specialist review you receive is an informational report, not a clinical assessment, prescription, or care plan.
• ClearSignal is not a substitute for care from your primary care provider or a licensed medical professional.

If you have questions about your health, a chronic condition, or symptoms you are experiencing, please consult a licensed healthcare provider.

2. Lab Testing

Lab testing offered through ClearSignal Health is facilitated through an independent, third-party ordering physician. ClearSignal does not directly order laboratory tests. By purchasing a lab panel:

• You authorize the independent ordering physician to place the lab order on your behalf.
• You acknowledge that the lab results are your personal health data and will be made available to you directly.
• You understand that ClearSignal's specialist review is based on the results provided and does not include physical examination, medical history review, or clinical evaluation.

Lab results and the specialist review are not reviewed in real time and do not constitute urgent or emergent clinical care. Do not use ClearSignal for time-sensitive health concerns.

3. No Patient-Provider Relationship

Your use of ClearSignal Health — including purchasing a lab panel, receiving a written specialist review, or any communication with ClearSignal — does not establish a patient-provider relationship of any kind. This is true regardless of the credentials of the specialist preparing your review.

Erica Finnan, MSN, APRN-C, is acting in an educational and informational capacity only through ClearSignal Health. Her involvement does not constitute clinical care, and she does not assume responsibility for your medical management through this service.

4. Scope of Specialist Review

The written specialist review included with your ClearSignal lab panel is a personalized informational report that may include:

• Explanation of your lab results in the context of functional and optimal reference ranges.
• General lifestyle, nutrition, and supplement considerations based on published research.
• Suggestions for further evaluation to discuss with your healthcare provider.

The specialist review does not include:

• Diagnosis of any medical condition.
• Prescription of any medication or controlled substance.
• A formal treatment plan or clinical recommendations.
• Ongoing monitoring or follow-up care.

Supplement suggestions, if included, are general and educational. You should consult with your healthcare provider before beginning any new supplement regimen, particularly if you have existing medical conditions or take prescription medications.

5. Refund Policy

Due to the nature of our service, which involves facilitated lab ordering and personalized specialist review preparation, all sales are final. We do not offer refunds once a lab order has been placed. We do not issue refunds based on dissatisfaction with individual lab results or the content of the specialist review.

6. Eligibility

ClearSignal Health services are available to adults 18 years of age or older who are residents of the United States. By using our services, you confirm that you meet these eligibility requirements.

ClearSignal is not intended for use by individuals who are pregnant, currently under active treatment for a serious medical condition, or who require urgent or emergent medical care. Please consult your healthcare provider if you are unsure whether ClearSignal is appropriate for your current health situation.

7. Emergency Situations

ClearSignal Health is not an emergency service. If you believe you are experiencing a medical emergency, or if your lab results reveal a critically abnormal value, do not wait for a specialist review. Call 911 or go to your nearest emergency room immediately.

If your lab results contain a critically abnormal value that may require urgent attention, the independent ordering physician may attempt to notify you directly. This does not constitute an ongoing clinical relationship.

8. Privacy and Data

Your health information is handled in accordance with our Privacy Policy, available at [INSERT PRIVACY POLICY URL]. We take the privacy of your health data seriously and do not sell your personal information to third parties.

By using ClearSignal Health, you consent to the collection and use of your information as described in our Privacy Policy.

9. Limitation of Liability

To the fullest extent permitted by applicable law, ClearSignal Health, Finnan Wellness LLC, and their officers, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services, including but not limited to reliance on the specialist review, lab results, or any informational content provided.

Our total liability to you for any claim arising from use of ClearSignal Health shall not exceed the amount you paid for the specific service giving rise to the claim.

10. Modifications to Terms

We reserve the right to update these Terms at any time. Material changes will be communicated via the ClearSignal website. Your continued use of our services following any update constitutes acceptance of the revised Terms.

11. Governing Law

These Terms are governed by the laws of the State of Ohio, without regard to conflict of law principles. Any disputes arising under these Terms shall be resolved in the appropriate courts of Ohio.
