What is lipoprotein(a)?
Lipoprotein(a) is an LDL-like particle with an additional protein, apolipoprotein(a), attached to it. That extra protein makes the particle both atherogenic and prothrombotic — it contributes to plaque formation and interferes with normal clot breakdown.
Your Lp(a) concentration is roughly 80–90% genetically determined. It is largely set at birth, changes little with diet or exercise, and therefore only needs to be measured once in most adults.
Why does it matter?
Roughly one in five people carries an elevated Lp(a). It is an independent risk factor for heart attack, stroke, and calcific aortic valve disease — independent meaning it adds risk even when LDL cholesterol, blood pressure, and everything else look reassuring.
Knowing it changes decisions. An elevated Lp(a) is a common reason to be more aggressive about the risk factors that can be modified: ApoB, blood pressure, insulin resistance, and smoking.

Why might it be missed on routine bloodwork?
Because Lp(a) is not readily modified by lifestyle and historically had no targeted therapy, it was rarely screened. Guidance is shifting toward measuring it at least once in every adult, but standard panels have not caught up, and it is almost never included in an annual physical.
What can cause it to be high?
- Genetics — by far the dominant factor, through variation in the LPA gene
- Ancestry-related differences in average concentration
- Chronic kidney disease, especially nephrotic syndrome
- Hypothyroidism, which can modestly raise levels
- Menopause, associated with a modest rise in some women
- Acute inflammation, which can transiently elevate a single measurement
What can cause it to be low?
A low lipoprotein(a) is simply a favorable inherited variant and does not require attention. Low values are occasionally seen with hyperthyroidism or significant liver disease, since the liver produces the particle.

What is a normal lipoprotein(a) level?
Levels below roughly 75 nmol/L (about 30 mg/dL) are generally considered normal, with risk rising progressively above that. Units differ between laboratories, so compare results in the same units.
How often should Lp(a) be tested?
For most adults, once is enough, because the level is largely genetic and stable. Repeat testing is mainly considered if the first result was drawn during acute illness or if thyroid or kidney function has changed substantially.
Can diet or exercise lower Lp(a)?
Not meaningfully. Lifestyle changes are still valuable because they lower the risk factors that compound with Lp(a) — ApoB, blood pressure, insulin resistance, and inflammation.
