ApoB: The Cardiovascular Marker Worth Asking For
ApoB counts the actual number of atherogenic particles in your blood. When LDL cholesterol and particle count disagree, ApoB is usually the one telling the truth.

What is ApoB?
Apolipoprotein B is a structural protein found on every atherogenic lipoprotein — LDL, VLDL, IDL, remnants, and lipoprotein(a). Each of those particles carries exactly one ApoB molecule, so measuring ApoB is effectively counting the particles capable of entering the artery wall.
A standard lipid panel reports how much cholesterol those particles are carrying. ApoB reports how many of them there are. Because atherosclerosis is driven by particle entry rather than cholesterol mass, the count is the more direct measure.
Why does it matter?
People with the same LDL cholesterol can carry very different particle numbers. Someone with insulin resistance often has many small, cholesterol-poor LDL particles: a reassuring LDL-C with a high ApoB. That discordance is common, and it is exactly the group whose risk a standard panel underestimates.
Risk tracks the number of particles that can get into the artery wall — not the cholesterol they happen to be carrying.

Why might it be missed on routine bloodwork?
Most screening guidelines and insurance pathways still center on total cholesterol, LDL-C, HDL-C, and triglycerides. ApoB is inexpensive and standardized, but it is not part of the default panel, so it is typically added only when specifically requested or when risk is already recognized.
What can cause ApoB to be high?
- Genetic conditions such as familial hypercholesterolemia
- Insulin resistance and metabolic syndrome, which increase small dense LDL particles
- Diets high in saturated fat in susceptible individuals
- Hypothyroidism, which reduces LDL receptor clearance
- Kidney disease, particularly nephrotic-range protein loss
- Excess alcohol and high triglyceride states
What can cause ApoB to be low?
A low ApoB is generally favorable from a cardiovascular standpoint and is expected with lipid-lowering therapy or a genetically low-particle phenotype. Very low values can occasionally accompany malnutrition, malabsorption, hyperthyroidism, liver disease, or rare inherited disorders of lipoprotein production, and should be interpreted alongside the rest of the panel.

What is a good ApoB level?
Targets are individualized by overall risk. Many prevention-focused clinicians aim below roughly 80 mg/dL for average-risk adults and considerably lower when cardiovascular disease, diabetes, or elevated lipoprotein(a) is present. Laboratory ranges are broader because they describe the population rather than an optimal target.
Do I need to fast for an ApoB test?
ApoB is relatively stable and does not strictly require fasting, though many panels are drawn fasting so that glucose, insulin, and triglycerides on the same draw remain interpretable.
Is ApoB better than LDL cholesterol?
ApoB is generally the more direct measure of atherogenic particle burden and tends to predict risk at least as well as, and often better than, LDL-C — particularly in people with insulin resistance or high triglycerides.
