Start with the question, not the test list

The most useful panel is the one built around what you are actually trying to learn. Screening for disease you may already have, understanding symptoms you are living with, and estimating risk decades out are three different jobs, and they call for different markers.

  • Am I developing something quietly? — metabolic, cardiovascular, and inflammatory markers
  • Why do I feel this way? — thyroid, hormones, iron, and nutrient status
  • What is my long-term risk? — ApoB, lipoprotein(a), fasting insulin, hs-CRP

What a standard annual physical usually covers

A typical wellness draw includes a complete blood count, a comprehensive metabolic panel, a standard lipid panel, and often a TSH and hemoglobin A1C. That combination is genuinely valuable — it detects anemia, kidney and liver problems, established diabetes, and gross thyroid disease.

Its limitation is design, not quality. These tests are built to identify disease that has already arrived. They are not built to detect the physiology that precedes it by five to fifteen years.

How to build a panel that is worth the draw

  1. 1.Keep the foundation: CBC, comprehensive metabolic panel, lipids, A1C.
  2. 2.Add the metabolic early-warning layer: fasting insulin and fasting glucose, so HOMA-IR can be calculated.
  3. 3.Add the cardiovascular layer: ApoB, and lipoprotein(a) once in your lifetime.
  4. 4.Add the inflammatory layer: hs-CRP, ideally when you have been well for a few weeks.
  5. 5.Add the thyroid layer if you have symptoms or family history: Free T3, Free T4, Reverse T3, TPO antibodies.
  6. 6.Add nutrient and hormone markers relevant to your age, sex, and symptoms.
  7. 7.Fast 8–12 hours, draw in the morning, and repeat under the same conditions so trends are real.
One well-designed draw usually answers more than three fragmented ones. The value is in seeing the markers together, because they interpret each other.

The step that matters most is interpretation

Ordering the right tests is only half of it. Reference ranges describe a broad population rather than an optimal state, and a result can sit comfortably inside a range while being far from where you feel and function well.

The goal isn't to catch your disease. It's to make sure it never gets that far.

Frequently asked questions

How often should I get blood work done?

For most healthy adults, an annual comprehensive panel is reasonable, with more frequent rechecks — often three to six months — after a meaningful change in diet, training, medication, or supplementation, so you can see whether the change worked.

Do I need to fast before blood tests?

Fast 8–12 hours if the panel includes insulin, glucose, or triglycerides. Water is fine. Keeping the conditions the same each time makes year-over-year comparison meaningful.
