Ferritin reflects stored iron — and it is one of the clearest examples of a marker where being technically in range and being adequate are not the same thing.

What is ferritin?

Ferritin is the protein that stores iron inside cells. Serum ferritin acts as a proxy for how much iron your body has banked — the reserve behind hemoglobin, oxygen transport, energy production, thyroid hormone conversion, and hair growth.

Why does it matter?

Iron stores deplete long before hemoglobin falls. A person can have a completely normal complete blood count and still be functionally iron deficient, with fatigue, breathlessness on exertion, hair shedding, restless legs, and poor exercise recovery.

This matters most for menstruating women, endurance athletes, frequent blood donors, people with gastrointestinal conditions, and anyone with a plant-predominant diet.

Why might it be missed on routine bloodwork?

Ferritin is not part of a standard CBC or metabolic panel, so iron status is often assessed only by hemoglobin — the last thing to change. And because the reference range starts near 12 ng/mL, a result of 15 is reported as normal, even though most people with symptoms improve only well above that.

What can cause ferritin to be low?

  • Heavy or prolonged menstrual bleeding
  • Inadequate dietary iron, or low bioavailability from plant sources
  • Malabsorption — celiac disease, inflammatory bowel disease, low stomach acid, or prior bariatric surgery
  • Gastrointestinal blood loss, which always warrants evaluation
  • Pregnancy, breastfeeding, and rapid growth
  • Endurance training and frequent blood donation

What can cause ferritin to be high?

  • Inflammation or infection — ferritin is an acute-phase reactant and rises independently of iron
  • Metabolic dysfunction and fatty liver disease
  • Excess alcohol intake
  • Hereditary hemochromatosis or iron overload
  • Iron supplementation, particularly when unmonitored
Because inflammation raises ferritin, a normal-looking ferritin alongside an elevated hs-CRP can mask true iron deficiency. Iron saturation and transferrin help resolve the picture.

Can ferritin be normal and I am still iron deficient?

Yes. Inflammation raises ferritin independently of iron stores, so a person with active inflammation can be iron deficient with a ferritin that appears adequate. Checking iron saturation and hs-CRP together clarifies this.
