What Blood Tests Should Women Over 40 Get?

Why 40 changes the picture
Perimenopause typically begins in the late 30s or 40s, often a decade before the final period. Progesterone tends to decline first, estradiol becomes erratic rather than simply low, and cycles can stay regular while symptoms are already significant.
At the same time, muscle mass declines gradually, insulin sensitivity tends to fall, thyroid autoimmunity becomes more common, and cardiovascular risk begins to rise. These are separate systems that produce overlapping symptoms — which is exactly why testing them together matters.

The value of a wide panel here is not that everything will be abnormal. It is that you can see which system is actually drifting, and stop guessing.

When in my cycle should hormones be tested?
If you are still cycling, progesterone is most informative roughly seven days after ovulation — around day 19–22 of a 28-day cycle. Estradiol and FSH are often drawn on day 3. If cycles are irregular or have stopped, timing matters less and the pattern across markers matters more.
Is an at-home hormone test accurate for women over 40?
Blood testing through a laboratory draw is accurate regardless of who ordered it. What varies is which markers are included and how well the results are interpreted — a hormone value without cycle context and without thyroid and metabolic markers alongside it is easy to misread.
Do I need testing if my periods are still regular?
Regular cycles do not rule out perimenopausal change. Progesterone often declines while cycle length is still normal, and thyroid or iron issues can produce identical symptoms.
