You had your hormones checked. Your testosterone came back "normal." So why are you still dealing with low libido, fatigue, brain fog, loss of motivation, reduced strength, or just not feeling like yourself?
One possible piece of the puzzle is a lab marker that doesn't always get much attention: sex hormone-binding globulin, or SHBG. SHBG can dramatically change how we interpret hormone levels — particularly testosterone.

What Is SHBG?
SHBG stands for sex hormone-binding globulin. It is a protein produced primarily by your liver that binds to sex hormones circulating in your bloodstream, particularly testosterone and estradiol.
Think of SHBG like a protective transport case for your hormones. Hormones circulating freely in your bloodstream are available to interact with tissues. But when a hormone is locked tightly inside that transport case, it isn't as readily available to those tissues.
SHBG binds especially strongly to testosterone. That means someone can have plenty of testosterone circulating in the bloodstream, but if a large percentage of it is tightly bound to SHBG, the amount available to tissues may be much lower. This is the difference between total testosterone and free testosterone.
Total Testosterone vs. Free Testosterone: What's the Difference?
Total testosterone tells us the total amount of testosterone circulating in the blood — including testosterone that is bound to proteins. Free testosterone represents the small fraction that is circulating unbound.
This distinction can become especially important when SHBG is elevated. Imagine two women who both have a total testosterone of 35. On paper, their testosterone levels look identical. But one has an SHBG of 50 and the other has an SHBG of 150. Those women may have very different amounts of free testosterone available to their tissues.
Why Does High SHBG Matter?
When SHBG is significantly elevated, more testosterone can become tightly bound and less may remain available to tissues. This can sometimes create an interesting laboratory pattern:
Normal total testosterone + high SHBG + low free testosterone
You may have been told your testosterone is "normal" because the total testosterone falls within the laboratory reference range. But that's not necessarily the whole story. If SHBG is elevated, looking at free testosterone and the broader clinical picture can provide important additional information.
What Are the Symptoms Associated With High SHBG?
SHBG itself doesn't necessarily cause symptoms. Rather, symptoms may occur because of the hormonal environment associated with an abnormal SHBG level — including reduced bioavailable testosterone. Depending on the individual, symptoms may include:
- Low libido
- Fatigue or decreased stamina
- Brain fog
- Difficulty concentrating
- Decreased motivation
- Lower mood
- Reduced muscle mass
- Difficulty maintaining strength
- Changes in overall sense of well-being
These symptoms are not specific to SHBG or testosterone deficiency. Thyroid dysfunction, iron deficiency, sleep problems, nutrient deficiencies, chronic stress, insulin resistance, perimenopause, and many other conditions can produce similar symptoms.
What Causes High SHBG in Women?
If SHBG is persistently elevated, the better question isn't "What can I take to lower it?" It's "Why is the liver producing more SHBG in the first place?" There are several possibilities.
1. Estrogen
Estrogen stimulates the liver to produce SHBG. This is particularly important with oral estrogen, because medication taken orally passes through the liver before entering systemic circulation. This "first-pass" liver exposure can significantly increase SHBG. Naturally produced ovarian estrogen can influence SHBG as well, although the effect is generally less dramatic than with oral estrogen.
2. Birth Control Pills
Combined oral contraceptives containing estrogen can significantly increase SHBG. In some women, SHBG may remain elevated for a period of time even after discontinuing birth control, although the degree and duration vary between individuals.
3. Thyroid Hormones
Your thyroid and sex hormones do not operate independently. Thyroid hormones influence SHBG production, and hyperthyroidism is a well-established cause of elevated SHBG. Excessive thyroid hormone replacement may also contribute in some patients.
4. Nutrition and Calorie Restriction
Low calorie intake, undernutrition, and certain eating patterns are associated with higher SHBG. If someone is chronically dieting, significantly restricting calories, losing substantial weight, or not consuming adequate nutrition, that context matters. Adequate protein and overall energy intake are part of the bigger hormonal picture.
5. Liver Health
SHBG is primarily made in the liver, so liver function and liver disease can affect SHBG concentrations. An abnormal SHBG is therefore another reason to consider overall metabolic and liver health rather than viewing it strictly as a reproductive hormone problem.
6. Alcohol
Alcohol consumption has been associated with alterations in SHBG levels, and the relationship can depend on the amount consumed and its effects on liver and hormonal function. For someone with persistently elevated SHBG, alcohol intake is worth factoring into the overall picture.
7. Aging and Genetics
SHBG can change with age, and genetics also influence an individual's baseline level. Sometimes a person's SHBG simply tends to run higher than someone else's. That makes trends over time particularly helpful.
What About Insulin and SHBG?
SHBG also has an interesting relationship with metabolic health. In general, higher insulin levels and insulin resistance are associated with lower SHBG, while lower SHBG is commonly seen with obesity, type 2 diabetes, and metabolic syndrome.

Low SHBG can result in a greater proportion of testosterone being available in its free form. In women, this may sometimes accompany symptoms of androgen excess such as acne, oily skin, facial hair growth, scalp hair thinning, or irregular menstrual cycles. Again, SHBG is not a diagnosis. It is a clue.
What Is an Optimal SHBG Level?
This is where individualized interpretation becomes particularly important. Laboratory reference ranges vary based on the laboratory, assay, age, and sex. Rather than chasing a single "optimal" number, SHBG is best evaluated alongside symptoms, medical history, and other laboratory markers.
As a general clinical framework, ranges you may see discussed include approximately:
- Men: 20–60 nmol/L
- Women: 30–135 nmol/L
However, being inside — or outside — one of these ranges does not automatically mean that something is right or wrong. Context matters more than chasing a single number.
Should I Have My SHBG Tested?
Not everyone needs SHBG testing. However, it can be particularly helpful when someone's symptoms don't seem to match their total testosterone level. For example, your total testosterone looks normal, but you're experiencing symptoms potentially consistent with low androgen availability.
SHBG can also be useful when evaluating people who:
- Use or previously used oral contraceptives
- Take oral estrogen
- Are receiving hormone replacement therapy
- Have thyroid disease
- Have symptoms despite apparently normal testosterone levels
- Have unexpectedly high or low testosterone results
- Have metabolic dysfunction or insulin resistance

Can You Lower High SHBG Naturally?
Sometimes — but the first question should be why SHBG is elevated. Trying to force SHBG down without understanding the cause misses the point. If inadequate calorie or protein intake is contributing, correcting nutrition may help. If excessive thyroid hormone is contributing, thyroid management needs to be addressed with the prescribing clinician. If oral estrogen is significantly raising SHBG, the route of estrogen administration may be worth discussing with your clinician. If alcohol or liver health is contributing, those factors deserve attention.
In other words: understand the person and the underlying physiology — don't just fixate on the SHBG number.
What About Supplements for High SHBG?
You'll find plenty of articles online recommending supplements such as boron, zinc, magnesium, vitamin D, DIM, stinging nettle, omega-3 fatty acids, B vitamins, and vitamin E for SHBG. Some of these nutrients may be appropriate for certain people, particularly when a deficiency or another specific indication exists. But stacking a supplement regimen simply because SHBG is elevated isn't the answer.
Supplements have biological effects. Dose matters. Medications matter. Medical history matters. Other supplements matter. Understanding your own results — and what they mean in context — is far more useful than throwing a long list of supplements at one abnormal laboratory value.
Lifestyle Still Matters
Supporting healthy hormone function starts with the same foundations that support almost every other system in the body:
- Eat adequate calories and protein for your body's needs
- Include plenty of plants and fiber
- Incorporate cruciferous vegetables regularly
- Minimize or eliminate alcohol
- Maintain healthy blood sugar and insulin sensitivity
- Walk and remain active throughout the day
- Incorporate regular Zone 2 cardiovascular exercise
- Resistance train several times per week
- Maintain a healthy body composition
- Prioritize consistent, restorative sleep
- Address chronic stress and recovery
None of these habits is a magic treatment for SHBG. Together, however, they create a healthier metabolic and hormonal environment.
A Lab Result Is More Than a Reference Range
This may be the most important takeaway. It has become easier than ever to order hormone labs. But getting a result and understanding what it means aren't the same thing. A number can fall neatly inside a reference range and still leave you without answers — because that number is entering an incredibly complex biological system.
