Perimenopause can begin a decade before your final period, and hormone levels swing rather than simply fall. Testing well means testing the right markers at the right time.

What actually happens in perimenopause
Perimenopause is not a steady decline. Ovulation becomes less consistent, so progesterone production falls first and most reliably. Estradiol, meanwhile, becomes erratic — often surging higher than in earlier decades before eventually falling.
That volatility explains why symptoms can be severe while cycles remain regular, and why a single hormone measurement can look entirely unremarkable.

What blood tests can and cannot tell you
Perimenopause is a clinical diagnosis based on age, cycle change, and symptoms. No blood test confirms it definitively, and normal hormone levels do not exclude it.
What testing does well is rule other things in or out — thyroid disease, iron deficiency, insulin resistance, anemia — and establish a baseline before any therapy is considered.
Frequently asked questions
Can a blood test confirm perimenopause?
Not definitively. FSH and estradiol fluctuate substantially month to month during the transition. Testing is most valuable for establishing a baseline and identifying other conditions that produce the same symptoms.
What if my cycles have already stopped?
Cycle timing no longer applies, so hormones can be drawn on any day. A morning fasting draw keeps the metabolic and thyroid markers on the same sample interpretable.
Is saliva or urine testing better than blood?
Serum testing is the most standardized and widely validated approach for the markers discussed here. Other methods answer different questions and are best used alongside, rather than instead of, blood testing.
